A stay in the skilled nursing facility (SNF) is now common among frail old people after hospitalization. The SNF offers round the clock nursing care and ready access to physical therapy, but tends to have minimal physician care and little emphasis on advance care planning. It is sometimes used inappropriately as a site for terminal care in lieu of home hospice.
